Trust Agreement

Prerequisite paperwork to join the Trust. The Agreement and Resolution must be adopted by the Member Governing Body. A checklist is included.

Representative Designation Form

Required for all members to designate a member representative and alternative representatives for your department. Only the designated member representative or alternate representative may cast a vote at a meeting of the Trust membership on behalf of your organization.
